When it is needed

Irreversible pulpitis
Lingering pain to hot or cold, or pain that wakes you at night, usually means the nerve is inflamed beyond recovery.

Abscess or infection
Swelling, a gum boil or tenderness on biting indicate infection at the root tip that antibiotics alone will not resolve.

Retreatment
A previously treated tooth that still hurts or shows a shadow on the X-ray can often be reopened, re-cleaned and sealed again.

Trauma or deep decay
A fractured or heavily decayed tooth may expose the nerve; treating it early keeps the root and avoids an implant.

What the appointment involves
- Step 1
Diagnosis
Sensitivity tests and a radiograph, sometimes a small-field CBCT, confirm which tooth is responsible and whether the canal anatomy is complex.
- Step 2
Cleaning the canals
Under anaesthetic and rubber dam the canals are cleaned, shaped and disinfected. Most single-visit cases take 60–90 minutes and are painless.
- Step 3
Filling and sealing
The canals are filled three-dimensionally and a radiograph confirms the seal. Infected cases may need a second visit with a medicated dressing in between.
- Step 4
Protecting the tooth
A root-treated back tooth needs cuspal coverage, an onlay or crown, to stop it fracturing. We normally fit this in the same visit to Tirana.
Aftercare
- Mild tenderness for a few days is normal; ordinary painkillers are usually enough.
- Avoid chewing hard food on the tooth until the permanent restoration is in place.
- Contact us if swelling or severe pain appears, this is uncommon and easily managed early.
- Have a follow-up radiograph after 6–12 months to confirm the bone at the root tip has healed.

- Is a root canal painful?
- The treatment relieves pain rather than causing it. The tooth is fully anaesthetised, and most patients say it feels like having a large filling.
- How many visits does it take?
- Many teeth are completed in one visit. Infected or previously treated teeth may need two appointments a few days apart, which still fits inside a short stay in Tirana.
- Do I really need a crown afterwards?
- For molars and premolars, yes, a root-treated tooth is more brittle and cuspal coverage dramatically reduces the chance of fracture. Front teeth can often be restored with bonding alone.
- Is it better to extract and place an implant?
- Keeping your natural tooth is almost always preferable when the root is sound. We only recommend extraction when the tooth is fractured, badly decayed below the gum or unrestorable.
